About the Opportunity Sunrise Health Service is currently seeking a qualified Diabetes Educator to join their team in Katherine, NT on a full-time basis. Reporting to the Dietitian/Nutritionist Cluster Leader, you will be part of a multidisciplinary team that includes dieticians, exercise physiologists and health promotion officers. All team members travel regularly to the nine remote clinic sites, which are all located in remote communities in the eastern Katherine region of the NT. Although this role will be clinically based, it will have a strong focus on interactive community engagement and proactive health promotion. Your day-to-day duties will include (but not be limited to): Collaborating with, and coordinating health-centre and specialist staff to ensure the delivery of appropriate care to patients for diabetes and associated conditions. Provide clinical leadership to other health professionals including Remote Area Nurses (RANs), Aboriginal Health Practitioners, medical and allied health professional staff in the area of diabetes management. Promote a multidisciplinary team approach to decision making and care provided.
