About the Opportunity

Due to a growing client base, Northside currently has a newly created opportunity for a full-time Clinical Relationship Coordinator to join their Community Services - Aged Care & Inclusion team in Canberra, ACT.

Reporting to the Senior Manager Aged Care and Inclusion, you will be responsible for coordinating both higher needs client engagement and entry into the Northside Aged Care and Inclusion programs.

With a mixture of case management and clinical duties, you will ensure all participants engaged with the program have a seamless, respectful and positive experience.

Your day-to-day responsibilities will include (but will not be limited to):

  • Implementing and facilitating an efficient process for the intake, assessment and onboarding of new clients in accordance with client-directed care (CDC) principles;
  • Conducting clinical reviews for existing and new clients, including medical history, falls risks etc.;
  • Providing a streamlined and timely collaborative planning and service engagement experience, to support client dignity and choice;
  • Supporting the monitoring and response of clinical risk notifications in a timely manner; and
  • Working closely with the Clinical Care Manager to ensure adherence to Aged Care Quality Standards and Clinical Governance policies.

About You

To be considered for this position you must be at minimum an Enrolled Nurse, ideally with relevant experience in the community sector (Aged Care etc.).

More specifically, you will need to have demonstrable experience in delivering clinical services to vulnerable clients with complex needs, and be comfortable in your ability to conduct clinical assessments and reviews.

While not essential, any demonstrated understanding of the issues associated with ageing will go a long way to ensuring your success within this client-facing role.

As an integral role within the community sector, your ability to understand and adhere to policy, process and scope of practice will be a critical factor in ensuring the provision of appropriate case management and clinical services.

With high-level emotional intelligence and communication skills, you will maintain effective relationships with all stakeholders, and use your empathic nature to relate to and support clients within a complex ageing environment.

Please note: You will need to either hold, or be willing to obtain, a Working with Vulnerable People Card and complete a Police Check. You will also need a current Australian Driver's Licence and access to a reliable vehicle.
