This is a great opportunity to work with clients with various chronic diseases living in the Yorke Peninsula region. Maintaining a close working relationship with a range of staff throughout the local health services including acute and community based staff, local strategic partners and the Program Manager, you will ensure the effective implementation and positive outcomes of the Better Care in the Community program in the local area. Promoting continuity and consistency of care in collaboration with the Better Care in the Community (BCIC) team, you will contribute to the implementation of practice changes and undertake service management responsibilities. This program aims to improve the support of clients with chronic conditions, reduce the number of potentially avoidable admissions, and decrease the length of stay in hospital.
You will play a pivotal role in providing health education, counselling and rehabilitation programs to improve the health outcomes of individual clients or groups and ensuring cooperative and productive working relationships with all members of the community health care team. Working within a multidisciplinary team in a relaxed rural location on the Yorke Peninsula, you will be able to enjoy a great work/life balance.
You will be registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and hold or be eligible to hold a current practicing certificate. With demonstrated experience working within rural or remote community settings, you will be skilled in working with stakeholders and in partnership with government agencies and the community to deliver mutual outcomes. This will include experience in the delivery or chronic condition self-management programs and skills in delivering group education.
