Make a real difference and feel like an important part of the Eyre & Far North Local Health Network Community Nursing Team. Joining the team, you will utilise a primary health care approach to identify and implement health promotion strategies, including illness prevention, health education, counselling, community development, advocacy and social action in the community. Providing proficient, person-centred clinical nursing care and individual case management to patients in the community, you will monitor patient care plans to ensure appropriate care outcomes are achieved on a daily basis. You will also be expected to monitor, promote and foster a culture centred on customer services, and a team approach to work practices and work operations, and facilitate staff development, client teaching and nursing research with a focus towards the maintenance of contemporary best practice. Maintaining cooperative and productive working relationships within all members of the community health team, you will support and work collaboratively with less experienced members of the nursing team.
You will be registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and hold or be eligible to hold a current practicing certificate. With demonstrated competence in community nursing practice in accordance with the appropriate standards, you will have effective communication skills, including problem solving, conflict resolution and negotiation skills. With the ability to work effectively within a multidisciplinary team and prioritise workloads, you will be creative, innovative and flexible when approaching issues within a community health setting.
This role is part of a job share role and can be based in either Wudinna or Elliston to provide community nursing services to that community.
