The Metro-Peri Urban Clinical Transfer Coordinator is accountable to the Program Manager, Country Referral Unit (CRU), and Country Health SA LHN for active participation in the Country Referral Unit.  Works closely with the other Clinical Support officers within CRU and works closely with the SA Metro Hospitals and SA Country Hospitals to co-ordinate the transfer of country and or metro patients to Peri Urban hospitals located across the Barossa Hills Fleurieu Region. The Metro-Peri Urban Clinical Transfer Coordinator works within a multi-disciplinary team framework, in collaboration with other health professionals, service providers and the community and maintains cooperative and productive working relationships within all members of the health care team. Supports and works collaboratively with other Clinical Support Officers The Metro-Peri Urban Clinical Transfer Coordinator may be responsible for supervision of less experienced professional officers, para-professional staff and students.

Accepts accountability for their own practice standards, activities delegated to others and the guidance and development of less experienced staff.

Provide support to the Nursing/Midwifery Nurse/Midwife Unit Manager or equivalent in the leadership of nurses/midwives in the ward/unit/service.

Employees in this role will undertake a portfolio within which they will:

  • Promote continuity and consistency of care in collaboration with other AN/MUM and the Nurse/Midwife Unit Manager or equivalent;
  • Assist the Nurse/Midwife Unit Manager or equivalent in the implementation of practice changes; and
  • Manage and promote a process to centralise flow of metro to peri-urban transfers ensuring a positive patient centred journey and appropriate outcomes at the peri-urban sites.
