Utilising your clinical nursing skills and experience, you will provide proficient, person-centred nursing services within the Aged Care Assessment Team (ACAT), which may include assessing client’s homes. Working within pre-determined guidelines, and in a multidisciplinary primary health care setting, you will assess clients, select and implement different therapeutic interventions and support programs, and evaluate progress. You will also assist and support the Nurse Unit Manager with management, clinical, and education activities, including planning and coordinating services from other disciplines.
