As a member of the multidisciplinary team, you will be responsible for the provision of specialist clinical nursing rehabilitation services to residents of the Trevor Parry Community Rehabilitation Centre (CRC). You will provide direct input and discipline specific expertise into the formulation of individual rehabilitation support plans in collaboration with residents, carers, referrers and other advocates and participants as identified and required, in order to enhance and facilitate individualised rehabilitation and recovery within the CRC and wider community.
As an advanced practitioner in a multidisciplinary team, you will be responsible for the planning, coordination, delivery and evaluation of clinical nursing rehabilitation services. You will provide direction, supervision and support for Community Rehabilitation Workers, overseeing high quality, evidence based and outcome driven rehabilitation and recovery services for the residents of the CRC. You will work across a range of environments including the Centre and larger community to assist and support the development of residents’ rehabilitation and recovery in partnership with their developing and existing networks.
You will be registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia, and hold or be eligible to hold a current practicing certificate. You must be enrolled in an approved Mental Health course or hold a qualification in Mental Health practice. With prior experience in management and leadership roles, you will be capable of giving supervision to student nurses, enrolled nurses and less experienced registered nurses. You will use your effective communication, problem solving, conflict resolution and negotiation skills to maintain a cohesive multidisciplinary team and resolve local or immediate nursing care problems.
