Working across a 24 hour, 7 days a week on call roster you will play a critical role in assessing and providing nursing services to clients in Adelaide with acute and chronic clinical needs. You will assess, plan, implement and evaluate your clients palliative care plans in collaboration with case managers and our multi-disciplinary health care team.
This role is primarily focused on the care of clients who have come from a hospital setting and moved back into their home. Therefore, experience working within the community or an acute hospital setting is essential.
