About the role

An exciting opportunity is now available for a Registered Nurse to join the vibrant SummitCare HomeCare team. This position will initially be on a casual basis with a view to permanent part time. We are seeking a passionate nurse who is passionate about supporting older people in the community.

As the Registered Nurse/Case Manager, you will be responsible for building strong customer relations, providing compassionate care in a nurturing and supportive home environment, completing in-home care assessments and client documentation. As part of this role, you will often be required to travel to and from client homes. You will report to and backfill the General Manager as needed, and work within a small dedicated and collaborative team based in Baulkham Hills.

About You

To be successful in this role, you will have:

  • Bachelor’s Degree in Nursing and current registration with AHPRA
  • Current Class C NSW Driver’s Licence and current criminal record check
  • Knowledge and understanding of the Home Care market and Aged Care operating environment
  • Proactive and innovative with exceptional customer service and communication skills
  • Demonstrated ability to achieve goals within set time frames and work efficiently to meet deadlines both independently and as part of a team
  • Proficient and competent in a range of relevant clinical skills including the composition of comprehensive clinical care plans
  • Willingness to mentor and support a wider team
  • Experience as a Registered Nurse working within the Home / Community Care sector is highly desirable
