You will provide pivotal coordination of patient care delivery in Aboriginal Health within the Southern Adelaide Local Health Network. The main focus of your role will be the leadership and coordination of the Aboriginal Health Clinical Services. These services deliver primary and intermediate health care via a range of preventive and health promoting programs in the community, along with a range of Nurse and General Practitioner-led chronic disease and general health management interventions at the Aboriginal Family Clinics and in the community/home setting as appropriate. The role also includes local resource management. Importantly, you will be responsible for ensuring culturally appropriate, equitable and comprehensive primary health care and the effective management of chronic disease for Aboriginal patients/clients. You will accept accountability for the outcomes of practices in the Aboriginal Health practice setting, addressing inconsistencies between practice and policy and developing team performance and positive work cultures in the interest of patient outcomes.
To be successful in this role, you will have significant experience in working in a leadership role with the Aboriginal community, Aboriginal and mainstream service providers. Having worked before in a clinic setting with clients presenting with complex chronic disease, you will bring the ability to work in a respectful and culturally appropriate manner with Aboriginal & Torres Strait Islander people and communities. Demonstrating flexibility, innovation and creativity which can be applied to the community setting, you will be confident in communicating and working with others to influence and earn the trust and respect of relevant parties gaining cooperation and support.
