Utilising your knowledge and advanced skills in the assessment and care of the older person, you will support and work with older people presenting to, or at risk of presenting to, the Emergency Department over a 7-day service. You will work on a rotational basis between the Single Point of Contact and the Residential Care Outreach Team roles, as part of a collaborative inter-disciplinary model of nursing, medical and allied health. Helping to identify older people who are frail/vulnerable or who are at risk of becoming frail, you will provide advice relating to hospital admission avoidance and early supported discharge. You will also assist with determining the appropriate care pathway both into the hospital and back into the community, and provide a single point of contact for residential aged care facilities (RACFs) and General Practitioners (GPs) to support decision making and give advice on services that could support the management of patients in the community where appropriate. You will also strive to ensure early discharge of RACF patients admitted to hospital and provide support to them and their families/carers during their hospital stay.
You will be registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia, and hold or be eligible to hold a current practicing certificate. With prior experience in management and leadership roles, you will be capable of giving supervision to student nurses, Enrolled Nurses and less experienced Registered Nurses. You will have a specialist level of knowledge, clinical and assessment skills required for the care of the older person and the ability to case manage complex patients.
